Registration Forms

In addition to the documents listed in Step 1, Westford also requires the following when registering your child to attend one of our schools:

• Proof of birth
  • Birth Certificate - Preferred document for enrollment. Please contact our District Registrar if one is not available.
• Proof of guardianship (Parent ID or Legal Guardianship papers)
• Proof of Residency Policy: (document from each column)
  • Document from Column A
  • Document from Column B
  • Document from Column C
  • WPS Annual Residency Update Submission Form (for families that rent or cohabitate)
• Proof of medical examination/health record (physical exam done in past year)
• Proof of immunization
  • Diphtheria, Tetanus, Pertussis (Dtap/DTP/DT/Td) 
  • OPV - Polio
  • Measles/Mumps/Rubella
  • Hepatitis B
  • Varicella/Chicken Pox  – unless exempted for medical or religious reasons
  • Lead test
• Most recent report card (DOES NOT apply to K, unless one is obtain from completing half of a year long K program outside of Westford Public Schools)

• Current, signed IEP or 504 plan
• Court records of custody and visitation rights
• WPS Rental Acknowledgement  - Please complete this if you rent, live with family or cohabitate in Westford. 

 If you rent or live with family in Westford, please provide the following at the time you register your child/children. 

 Please note: For as long as y our rent or live with family, the district will contact you annually requesting current rental documentation as long as your student(s) remain enrolled in Westford Public Schools. 

1. When a student and parent/caregiver lives with the owner of the property but does not hold formal lease, we require:
   1. Cohabitate Affidavit MOU 
   2. Parent Occupancy Affidavit
   3. Two (2) supporting documents from COLUMN B and COLUMN C found on the Westford Residency Regulation (R5115).
2. When the student's parent/caregiver rent property, we require:
   1. A copy of the lease
   2. Two (2) supporting documents from COLUMN B and COLUMN C found on the Westford Residency Regulation (R5115).
3. If no formal lease is available, we require:
   1. Landlord Affidavit
   2. Parent Occupancy Affidavit
   3. Two (2) supporting documents from COLUMN B and COLUMN C found on the Westford Residency Regulation (R5115).
4. ANNUAL RENTAL UPDATES REQUIRED:  Please submit the required documents using this WPS Annual Residency Update Google Form.
