Use this form to update your home address, personal email, or phone number. Note: Keeping this current ensures you receive important tax documents and district mailings.

Name Change Requirements

To update your legal name in our systems, please submit the Employee Information Form along with the following documents to the HR Office.

Important: We cannot process a name change until the update is reflected with the Social Security Administration.

Required Payroll & Federal Documents
1. New Social Security Card (Required by the IRS to match your W-2)
2. New W-4 (Federal)
3. M-4 (State) withholding forms
4. New I-9 Form (Federal Employment Eligibility Verification)
5. Updated Driver’s License
6. For DESE-Licensed Staff
DESE Profile Update: You must update your legal name via the ELAR Portal first.
1. Please provide a copy of your updated ELAR profile showing your new legal name to the HR Office.
Supporting Legal Documents
1. Please provide a copy of your Marriage Certificate, Divorce Decree, or Court Order for our records.
