CURRENT STUDENTS - for scholarships, recruitment, summer program and private school transcripts, please see your counselor.

This easy request form is for student transcripts to apply for driver licenses and state ID's.

DMV Requests must be made AT LEAST one week in advance prior to your DMV appointment, especially if you require them to be mailed.

PLAN AHEAD:
NO TRANSCRIPTS are processed through this page when the office is CLOSED for weekends and school breaks.

DMV DOCUMENTS REQUESTS IN SUMMER: Students needing DMV DOCS between July 1- Aug 9:
TRANSCRIPTS & PROOF OF ENROLLMENT LETTERS must be requested between
> 6/15/24 - 6/24/24 <
(PROOF OF ENROLLMENT forms are only good for 60 DAYS from date of letter.)

****This form will close for requests and NO documents will be processed after JUNE 27, 2024 at 12pm****

THE COUNSELING OFFICE IS CLOSED JULY 1 - AUGUST 9 each year, as well as the weeks of
DECEMBER, FEBRUARY and APRIL school vacations, school holidays and emergency snow days.
Last minute requests may not be accommodated.
